Chronic disease is a leading cause of death and disability in the United States. With an increasing demand for care and rising costs related to chronic disease, physicians need to be better trained to address chronic disease in a collaborative and cost-effective manner. With Wagner’s Chronic Care model as a framework, we used a literature review and Delphi survey to reach consensus on learning objectives for chronic disease prevention and management (CDPM) for undergraduate medical education. After two survey rounds with medical school faculty members that are part of the AMA Accelerating Change Consortium, we reached consensus on 11 core learning objectives. These learning objectives can be used as a guide for institutions interested in developing or enhancing their curriculum.
