A National Survey of Anesthesiology Residency Programs’ Milestones Implementation

Abstract

The Accreditation Council for Graduate Medical Education (ACGME) Milestones are foundational to competency-based medical education, providing a structured framework to assess resident progress across core competencies. With the introduction of Anesthesiology Milestones 2.0, programs are expected to adopt a more refined and clinically relevant evaluation system. This study used quantitative and qualitative analyses to explore how the Milestones are conceptualized and implemented across anesthesiology residency programs through a national survey, informed by previous findings from focus groups. The results emphasized previously observed inconsistencies among residency programs and highlighted challenges for implementation. This study suggests the need for continuing improvement and standardization of Milestones evaluations which has implications for other medical specialties that use Milestones evaluations.
