Many grant programs mandate a coordinated community response (CCR) model to be responsive to a community's needs and challenges. In an HVIP setting, a CCR draws on the expertise of community members, practitioners and local service providers who are knowledgeable of the community. While funding agencies acknowledge the importance of CCR models there is little discussion on what this looks like on the ground and in connection to data evaluations. This presentation will examine the development of a CCR in an HVIP setting and discuss why data evaluation is crucial for longevity and scalability of programs.
