Successes and Challenges Adapting the Trier Social Stress Test for Remote, Diverse, and fMRI Populations

Abstract

Disruptions to how the body responds physiologically to stress has long been recognized as a public health concern for children and adults. Stress reactivity is sensitive to psychosocial stressors, can be measured in the cortisol output of the hypothalamic–pituitary–adrenocortical (HPA) axis, and both underactivity and overactivity of the HPA axis has been associated with various physical and mental health problems. Measuring HPA axis function across development is of critical importance to understand how the developing body responds to and recovers from psychosocial stress. The Trier Social Stress Test for Children (TSST-C) is a gold standard, lab assessment of stress reactivity. However, this lab assessment can be difficult to conduct in younger children and those for whom a university setting is a barrier. Further, gaps remain in our understanding of what happens in the brain during stress reactivity tests, leaving us uncertain of mechanisms. This symposium showcases 4 adaptations to lab measures of stress reactivity which make the TSST more sensitive to differences in age, sex, and race, measure stress reactivity in an MRI, and a completely online TSST in response to COVID and the need for access. The discussion will revolve around the importance and feasibility of (1) being able to examine the brain during stress reactivity, and (2) bringing the measurement of stress reactivity to reach other populations through various methods. The discussion will also look to integrate how each of these adaptations into each other, providing a rich context for future research directions and practical application.
